The Congregational Health Network is a partnership between Methodist Le Bonheur Healthcare, congregations and other relevant community entities in the Memphis metropolitan area designed to address health issues by increasing the congregation’s and the hospital’s ability to care for their members and the surrounding community.


Monday, September 25

10:00 – 10:15 am  Welcome and Partner Introductions

10:15 – 12:30  Participant introductions and introduction to the Center of Excellence in Faith and Health.

Introductions of participants and staff. Who are you, your agency and what do you hope to learn? The concept of the Center of Excellence. CHN as an asset-based community partnership. Why CHN works in Memphis – Mission – Ministry – Market Share. Initial Group formation.

12:30 – 1:00  Lunch and conversation

1:00 – 1:40  System overview. Setting the stage for implementing and adapting your community health strategy, Strategic Plan development and Community Health Needs Assessment.                                                                                                  

2:00 – 2:10 pm  Break

2:10 – 3:45  Community Health Asset Mapping, Tracking and Technology. Identifying the assets, CHN data collection, CERNER, tracking the data and technology

3:45 – 4:00 Questions & Answers and End of Session
