Primary Care Access Network (PCAN), Florida Hospital, Orange County Florida Web site, is a dynamic collaborative among hospitals, local government, primary health care centers, community agencies, and other social services designed to improve the access, quality and coordination of health care services to the underinsured and uninsured populations of Orange County.

Camden Coalition of Healthcare Providers, Care Management Program that seeks to reduce unnecessary ED utilization in Camden, NJ, by intervening and directing appropriate outreach attention to those lacking consistent primary care, suffer from chronic illness, mental illness, and/or substance abuse.

imgs-AHC-logos-disparities-250Adventist HealthCare, Center on Health Disparities (MD, NJ) seeks to remove barriers to care, and create opportunities for health access, treatment by offering culturally competent education and training modules for health care professionals and staff, partnering with community organizations to make health care and services accessible to all members of the community and disseminating health disparities research and exemplary practices that address them.

Lutheran Healthcare Community Liaison Program, facilitates access to high-quality care by assigning a culturally competent liaison to serve the needs of three of its major patient groups—Orthodox Jewish, Arab, and Chinese—and provide culturally competent care.
